Extra notes: We run a successful Women's only gyms called "Women's Fitness Plus". We are introducing a new service, tailored nutrition plans under a separate brand "Complete Nutrition".

First we would logo to create a logo for Complete Nutrition and once we have agreed on this we would like to create a professional design for our questionnaire in which we will be email to clients to fill out.

Re Logo: We have a few Ideas but totally open to suggestions. Text logo and the colours we were thinking were Grey & Yellow. Our current (Women's Fitness Plus) logo is attached. I was thinking maybe replace Blue and Pink with Grey & Yellow and replace Women's Fitness with Complete Nutrition. Replace the plus symbol with maybe 360 degrees arrow (attached) as our new service is complete (so 360 degrees represent this well) nutrition. We would love a few versions and are open to suggestions from yourself.

Re Questionnaire: Attached in a word doc is the questionnaire. We would like the to be designed to look more professional and clear to understand. This will be sent via email and those receiving it will need to fill this out so the format will need to allow this.
